Certified Behavioral Health Case Manager (MHTCM) – MERIDIAN ASSOCIATES LLC SUITE1245 – Orlando, FL

Description:

Candidate will provide goal-oriented and individualized support through assessment, planning, linkage, advocacy, coordination and monitoring of service activities for our clients.

Duties:

1. Performs functions involved with assessment, including initial contact with the client, facility personnel, and parent/legal guardian (if client is a minor) regarding the client’s needs, presenting problems, and any other related issues.

2. Completes assessment of each client to include psychosocial history, mental status, behavior, family dynamics, educational/vocational performance, medical needs, and legal involvement.

3. Identify each client and family’s strengths as well as obstacles that impede success of meeting day to day coping.

4. Develops Service Plan that ensures access to services identified in the assessment as necessary to the client’s success in meeting their needs. The Service Plan includes goals relative to the medical, educational/vocational, mental health and substance abuse issues of the client. Identify barriers to achieving goals and contain an action plan to address barriers.

5. Conduct home visits based on individual client and family need ensuring client’s safety.

6. Provide referrals to community resources.

7. Advocate for the client; attend and testify at court hearings, case reviews, staffings and other meetings as required.

8. Provide support to children, families and caregivers

9. Participates in all client staffing to ensure that services in the Service Plan respond to the strengths and needs of the client.

10. Facilitates treatment team reviews with the client, therapist, and other members of the treatment team including family members.

11. Monitors client’s progress and needs through regular discussion with the client, therapist, designated school or vocational personnel, family, and other facility staff members.

12. Makes regular contact with client and parent or legal guardian (if client is a minor) to explain the services of case management in preparation for client’s discharge from facility if hospitalization/ institutionalization is involved.

13. Documents case management activities in chronological notes according to Meridian Associates policies and procedures. This documentation shall consistently refer to the goals and objectives specified in the client’s Service Plan and the progress or lack of progress toward these goals.

14. Provides other written status reports to facility or referral source as requested.

15. Ensures that all assessments, Service Plan reviews, transition plans and discharge summaries are reviewed and signed.

16. Stays abreast of current information in the field and resources available through training attendance as recommended by the supervisor.

17. Follows all Meridian Associates Policies & Procedures including Protected Health Information (PHI), HIPPA as required by Federal & state laws.

18. Attend mandatory monthly staff meetings.

19. Attend professional trainings and seminars.

20. Submission of documentation, billing and all documents according to Meridian Associates policies and procedures.

Qualifications:

Formal Education:

Bachelor’s degree or higher from an accredited college or university.

Content Specific Training:

Completion of an AHCA approved targeted case management training program. All training must have been completed within the last 10 years. Training MUST be completed within 90 days of initially providing Medicaid Services.

Related Work Experience:

2,000 hours providing mental health targeted case management services to Children and/or Adult Mental Health Target Group(s).

All experience must be paid, formal experience. Volunteer work and internships are not eligible for certification purposes.

Certifications:

Skills Necessary:

Florida Certification Board – MUST be completed within the first 180 days of employment.

Job Types: Full-time, Contract

Salary: $16.00 to $20.00 /hour
